Schedule a Physical Exam: • Please arrange for your child to have a physical exam with a licensed healthcare provider. 2. Submit the Completed Physical Form: • After the exam, ensure that the healthcare provider completes the school physical form. • Select "next" to submit the completed form by July 31. Note: • This requirement applies to all students and must be fulfilled annually to remain in compliance with Florida State School board Laws. • If your child has already completed a physical for another activity (e.g., dance), please provide that documentation to fulfill this requirement.
